Procedures and rules for applying sublicense for workers (Form 4)
Applicants must submit application with copy of ‘Sokongan Pas Kerja BUR555’ and ‘Borang 500’ from Labour Department including the copy of passport/IC.
Sublicense for forest workers is only valid for a period of 6 months.
Sulicense must be renewed 2 weeks before the expiry date for every forest workers. Failure to comply with the rules, action will be taken against the company according with the provision of the Forest Act.
Two passport-sized / IC photo are necessary only for new employee who previously had not given sublicense by the Department.
For the application of continuation of sublicense, the worker’s photo is no longer needed for a period of 3 years.
Previous sublicense must be returned before the new sublicense is issued.
A service charge of $2.00 will be charged for each sublicense issued for each employee.
